Job summary

Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will travel locally to client homes in Virginia, supporting families navigating Medicaid Waiver programs. The role emphasizes compassionate, organized service and community impact, with remote work possibilities from a laptop or tablet.

Candidate requirements include at least 2 years in disability or elderly support, no degree required, a valid driver's license, and the ability to pass a background check.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or elderly.
• No degree required; heart and experience prioritized over formal education.
• Valid driver's license and reliable vehicle required (no client transport).
• Ability to pass a background check and provide 2 professional references.
• Tech-savvy with email, documents, spreadsheets, and video calls.

Responsibilities

You'll spend your days connecting with families, helping them navigate Virginia's Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and making sure those you serve can live safe, healthy, and independent lives.

What You'll Do

• Hit the road (locally!) to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.

What We're Looking For

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver's license & reliable vehicle (no client transport).
• Great communication - both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points if you've got Person-Centered Thinking/Planning training.

Based on the position's expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
