Job summary

Join Moms In Motion - A Virginia Service Facilitation Provider and become a Service Facilitator (SF) Case Manager. You’ll visit families in their homes, help them navigate Virginia’s Medicaid Waiver programs, and support safe, healthy, independent living.

We value compassion, organization, and community focus. You’ll work locally with families, coordinating plans of care, and using a laptop to stay connected with clients, all while earning $40k–$55k annually with solid benefits.

Qualifications

• 2+ years of experience supporting individuals with disabilities or elderly (preferred).
• No degree required.
• Proficient with email, documents, spreadsheets, portal navigation and video calls.
• Must have a valid driver’s license and reliable vehicle.
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.

• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.

• A valid driver’s license & reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check + provide 2 professional references.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
