Job summary

Reach For Your Potential, Inc. is seeking a Service Coordinator to be the primary contact for all services delivered.

You will supervise staff, coordinate individual service plans, and maintain regular contact with members, families, and partnering agencies. This on-site role requires strong communication, organizational skills, and the ability to manage transportation logistics and documentation to IAC standards.

Qualifications

• Bachelor’s degree in the social or behavioral sciences with at least one year post-degree experience in human services, or
• High school degree with three years of post-degree experience in human services, or
• A combination totaling five years of education/experience with at least one year in the outlined area.

Responsibilities

• Supervise staff (Facilitators and Direct Care) including hiring, promotion, training, evaluation, discipline, and termination.
• Direct monthly meetings and recordkeeping for staff.
• Assist the Financial Department with member finances and staff timesheets.
• Assist the Medical Department with transportation and staff accompaniment to medical appointments and communicate health needs.
• Assist the QA Department with maintaining member documentation to IAC standards.
• Assist Schedule Clerks with scheduling issues, open shifts, and daily phone check-ins.
• Coordinate individual service planning for each member and review semi-annually with IDT.
• Maintain regular contact with members, parents, guardians, and other agencies.
• Transport members and coordinate transportation for activities; ensure goals and supports align with plans.
• Ensure member homes are maintained, organized, and stocked with supplies.
• Make frequent visits to member locations to ensure home well-maintained.
• Coordinate moves for new members, transfers between RFYP locations, or discharges.
• Attend weekly professional staff meetings and Service Coordinator meetings; attend requested meetings.
• Complete member and agency paperwork in a timely, professional manner.
• Respond to concerns within 48 hours.
• Complete other duties as assigned.
