Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Service Coordinator to assess individuals' needs, advocate for your people, and develop and monitor Person Directed Plans. The role collaborates with service providers to support effective plans and to maintain confidentiality under HIPAA.

Qualifications

  • Bachelor's or advanced degree from an accredited college or university.
  • Or an associate degree in social/behavioral/health-related field and related work.
  • Or high school diploma with two years of relevant experience.

Responsibilities

  • Assess individuals' needs and develop and monitor Person Directed Plans.
  • Advocate for clients and collaborate with service providers for effective care.
  • Provide skills training, crisis prevention and management.
  • Maintain confidentiality and adhere to HIPAA regulations.
  • Write and document clearly with correct grammar.
  • Schedule and conduct meetings following state and company guidelines.
  • Respond to crisis interventions with minimal supervision.
  • Meet regularly with individuals to track progress and compliance.

Job description

The Service Coordinator will assess individuals' needs, advocate for yourpeople, and develop and monitor Person Directed Plans. The ServiceCoordinator will collaborate with service providers and agencies to supporteffective Person Directed Plans. The Service Coordinator will provideskills training, crisis prevention and management, and maintain a positiverelationship with individuals, family members, and service providers to supportsuccessful, person-centered outcomes. This compassionate, caring, andsupportive team member will contribute to the overall lifetime potentialsuccess of an individual living with intellectual and/or developmentaldisabilities and their state of well-being.

Youwill get to:
  • Assess individual needs and collaborate with themto develop and execute effective life management and Person DirectedPlans.
  • Provide relevant skills training to improve dailylife.
  • Collaborate with service providers and agencies tobe a liaison for effective care and positive outcomes.
  • Write and document articulately while usingcorrect grammar so that documentation is clear and valuable.
  • Schedule and conduct meetings that adhere to bothstate and company-provided guidelines.
  • Maintain compliance with legal requirements andcompany policies and procedures.
  • Respond to crisis intervention with minimalsupervision.
  • Consistently meet with individuals to maintainprogress and compliance.
  • Guard confidentiality with knowledge and adherenceto HIPAA regulations.
You'll definitely need:
  • A bachelor's or advanced degree from an accreditedcollege or university.

or

  • An associate degree in a social, behavioral, humanservice, or health-related field, such as psychology, social work,medicine, nursing, rehabilitation, counseling, sociology, humandevelopment, gerontology, educational psychology, or education

or

  • A high school diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
  • Minimum 1 year of experience working with individuals with developmental disabilities.
  • Maintain valid Texas state Driver's License and auto insurance.
  • Complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment
