Hill Country Mental Health and Developmental Disabilities Centers seeks a Mental Health Care Coordinator to assess needs, craft person-centered recovery plans, and support families dealing with mental health challenges. The ideal candidate will hold a relevant bachelor’s degree and pursue QMHP-CS credentialing, with a Texas driver’s license and ability to obtain

Qualifications

• Bachelor's degree in psychology, sociology, behavioral or human services; QMHP-CS credentialing eligibility.
• Valid Texas driver’s license and auto insurance.
• Must obtain PMAB, First Aid and CPR within 3 months of employment and maintain.
• Preferred: Experience with Child/Adolescent Needs and Strengths or resilience models.

Responsibilities

Job description

A Mental Health Care Coordinator will assess needs,develop treatment plans, and effectively collaborate with individuals and theirfamily members. The Care Coordinator will assess strengths and needsand, in collaboration with the individual, develop a person-centered recoveryplan. The Care Coordinator will provide skills training, crisisprevention/management, linkage, and referral to support goal progress forchildren and families living with mental health challenges. Thiscompassionate, caring, and supportive team member will contribute to the skillsimprovement, achievements, and overall potential success of a child and theirstate of wellbeing.

You willget to:

• Assess individual needs andcollaborate with them to develop and execute effective recovery and lifemanagement plans.
• Provide relevant skills trainingto improve daily life.
• Collaborate with serviceproviders and agencies to be a liaison for effective care and positiveoutcomes.
• Write and document articulatelywhile using correct grammar so that documentation is clear and valuable.
• Maintain compliance with alllegal requirements and company policies and procedures.
• Consistently meet withindividuals to maintain progress as well as compliance.

You’ll definitely need:

• A bachelor's degree from an accredited college or university in psychology, sociology, behavioral or human services, with the ability to be credentialed as a QMHP-CS.
• Maintain a valid Texas state Driver's License and auto insurance
• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
• Preferred: One year of experienceimplementing the Child and Adolescent Needs and Strengths Assessment or the Texas Resilience and Recovery Model for Adults or Children.
