Job summary

Hill Country Mental Health and Developmental Disabilities Centers in Kerr County seeks a Care Coordinator to assess needs, develop person-centered recovery plans, and help children and families access services. You will collaborate with families and providers to improve outcomes.

Responsibilities include documenting plans clearly, maintaining compliance, providing skills training, crisis planning, and coordinating referrals to support ongoing recovery and well-being.

Qualifications

• Bachelor's degree in social, behavioral, or human services is required or equivalent
• Eligible to be credentialed as a QMHP-CS
• Texas driver's license and auto insurance required

Care Coordinator (Kerr County MH Center)

Help us help others:

A Mental Health Care Coordinator will assess needs,develop treatment plans, and effectively collaborate with individuals and theirfamily members. The Care Coordinator will assess strengths and needsand, in collaboration with the individual, develop a person-centered recoveryplan. The Care Coordinator will provide skills training, crisisprevention/management, linkage, and referral to support goal progress forchildren and families living with mental health challenges. Thiscompassionate, caring, and supportive team member will contribute to the skillsimprovement, achievements, and overall potential success of a child and theirstate of well-being.

You willget to:

• Assess individual needs andcollaborate with them to develop and execute effective recovery and lifemanagement plans.
• Provide relevant skills trainingto improve daily life.
• Collaborate with serviceproviders and agencies to be a liaison for effective care and positiveoutcomes.
• Write and document articulatelywhile using correct grammar so that documentation is clear and valuable.
• Maintain compliance with alllegal requirements and company policies and procedures.
• Consistently meet withindividuals to maintain progress as well as compliance.

You'll definitely need:

• Bachelor's degree from an accredited college or university in social, behavioral, or human services with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
• Maintain a valid Texas state Driver's License and auto insurance.
• To complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment

Preferred -

• At least one year of experience implementing the Child and Adolescent Needs and Strengths Assessment or the Texas Resilience and Recovery Model for Adults or Children.

• Be available to manage crisissituations as they occur in sometimes off-scheduled hours (this istypically an infrequent occurrence)
• Employ excellent verbal andwritten communication skills to engage and report effectively.
• Maintain a well-organizedschedule while allowing room for flexibility and adjustments.
• Maintain accurate and timelycollaborative documentation.
• Sustain a calm demeanor whenunder pressure to meet deadlines and handle sometimes difficultsituations.
• Provide exemplary customerservice and care to individuals with their best interests at heart.
• Work autonomously to completeassignments within company and state parameters with a high level ofquality.
• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
