Job summary

UPMC Western Behavioral Health at Safe Harbor in Erie County seeks a Service Coordinator I to join our Housing Case Management team. You will coordinate mental health services, develop service plans, advocate for recovery and connect clients with community resources in a Housing First framework.

The role requires a Bachelor’s degree in a Human Services field, a valid driver’s license, and a commitment to safe driving. Some on-call coverage and a sign-on bonus are included to support applicants.

Qualifications

• Bachelor's degree in a Human Services field required.
• Valid driver's license and transportation ability essential.
• Experience with case management, mental health or housing unstable individuals preferred.
• Safe driving record required.

Responsibilities

• Coordinate mental health services and advocate for recovery.
• Develop and implement service plans with individuals, families and support systems.
• Facilitate interdisciplinary meetings to ensure needs are met and services are accessible.
• Provide support connecting individuals with internal and external resources.
• Maintain accurate records and participate in trainings on best practices.
