Job summary

Kern Regional Center is looking for a dedicated Service Coordinator/Case Manager to serve the community in Kern, Inyo and Mono Counties. The role entails coordinating services, advocacy, and IPP/PCP development, with progress reports and annual reviews for clients in traditional and Self-Determination programs.

The ideal candidate will have a BA/BS in a related field, be bilingual (Spanish/English), and demonstrate strong communication, organization, and computer skills, plus a valid driver’s

Qualifications

• BA/BS degree preferably human services or human behavioral science and two years of experience.
• Bilingual fluency in Spanish and English.
• Detail-oriented and highly organized.
• Strong computer skills with word processing, spreadsheets and presentations.
• Strong written and verbal communication skills.
• Must have valid driver’s license, DMV printout, and reliable transportation.

Responsibilities

• Provide overall service coordination and advocacy for consumers and families.
• Arrange services and coordinate for consumers and families.
• Develop and implement the consumer’s IPP and PCP; prepare progress reports and annual reviews.
• Include follow-up, reassessments and periodic progress reviews of the program.
• Seek community resources and provide information and referrals.
• Initiate the purchase of vendored services and monitor service delivery.
• Stay abreast of state/federal regulations and agency policies related to consumer services.
