Qualifications

• Bachelor’s degree in Social Work or related field, or a bachelor’s in Social Work with four years of relevant experience.
• Strong communication and collaboration abilities across families, service providers, and professionals.
• Valid driver’s license and ability to travel within assigned catchment areas.

Responsibilities

• Coordinate and facilitate services for infants and toddlers with developmental delays.
• Develop and implement IFSP plans with families and professionals.
• Travel for home visits and community-based assessments.
• Advocate for families and support transition planning after age 3.
• Maintain accurate records and documentation per regulations and policies.
• Collaborate with service providers to monitor child progress and outcomes.
