Job summary

Jobtailor seeks an experienced Care Coordinator to assess and coordinate care for members with chronic illnesses, disabilities, and behavioral health needs. You will travel to members' homes and collaborate with an interdisciplinary team to implement person-centered service plans.

Licensed professionals (RN or LSW/LCSW/LMHC) with at least 2 years of related experience and strong communication skills are preferred; behavioral health training may be required. Travel ~50% of the time.

Qualifications

  • Current RN/LCSW/LSW/LMHC license in the applicable state(s)
  • Minimum 2 years' experience with chronic illnesses, co-morbidities or disabilities in care coordination or similar role
  • State-specific certification may be required by contract
  • Strong communication and interpersonal skills
  • Ability to work effectively in an interdisciplinary team
  • Compassionate, patient-centered approach
  • Behavioral health training may be required

Responsibilities

  • Conduct assessments of members’ physical health, behavioral health, social services, and long-term needs
  • Identify high-risk members and those who may need alternative care or waiver programs
  • Obtain clinical data as directed by RN or care coordinator
  • Coordinate care for members with chronic illnesses, co-morbidities, and disabilities
  • Collaborate with the interdisciplinary team to develop person-centered service plans
  • Participate in team rounds
  • Collaborate with care coordinators to ensure effective care coordination
  • Travel to members’ homes and other locations to conduct assessments and provide support
  • Focus on clinical assessments and support for behavioral health needs

Job description

  • Coordinate care for members with chronic illnesses, co-morbidities, and disabilities
  • Collaborate with the interdisciplinary team to develop and implement person-centered service plans
  • Participate in team rounds
  • Collaborate with care coordinators to ensure effective care coordination
  • Travel to members’ homes and other locations to conduct assessments and provide support
  • Focus on clinical assessments and support for members’ behavioral health needs
Requirements
  • Current, unrestricted RN, LSW, LCSW, or LMHC license in applicable state(s) required
  • Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role; or any combination of education and experience providing an equivalent background
  • State-specified certification based on state law and/or contract may be required
  • Strong communication and interpersonal skills
  • Ability to work effectively as part of an interdisciplinary team
  • Compassionate and patient-centered approach to care
  • Behavioral health training may be required to meet contractual workforce requirements
  • 50% travel required
