Qualifications

Responsibilities

  • Supports nursing staff during medical assessments for new admissions and ongoing evaluations.
  • Monitors medication administration, records and MAR documentation; maintains resident documentation and care plans.
  • Coordinates the service plan, communicates with physicians or pharmacies on new orders and issues; follows up on hospital readmissions.
  • Coordinates appointments with onsite medical provider and assists residents with transportation.
  • Performs med cart audits and ensures meds are checked as needed.
  • Provides regular support to resident care needs and follow-up; ensures incident/fall reports are completed.
  • Seeks quality improvement activities focusing on wellness, clinical management and safety.
  • Supports hiring, training, and oversight of clinical care staff.
  • Maintains skin sheets, shower schedules, and charting; reviews call light times for prompt response.

Description

The Resident Services Coordinator provides support to department head managers and staff as it relates to community operation with specific focuses on resident directed services. Under the direction of the Clinical Director this position ensures the highest degree of quality care by planning and organizing the overall operation of our care services in accordance with resident needs, government regulations and our internal policies and procedures.

ESSENTIAL RESPONSIBILITIES

include the following. Other duties may be assigned.

  • Provides support to nursing staff conducting medical assessments of each new admission as well as ongoing resident evaluations. This role does not complete assessments unless trained to do so and delegated properly.
  • In a quality control role, monitors medication administration, medication records and medication documentation in the MARs. Maintains resident documentation, medical management documentation and care plan documentation as directed by supervisor.
  • Reviews the care coordination of all residents, coordinates the service plan, activates appropriate resources and communicates with the physician or pharmacy regarding new medication orders, changes in orders and any medication issues. Follows up on residents admitted to the hospital and completes necessary paperwork for a re-admission to the community after hospitalization or rehab. Addresses any resident issues/concerns with families.
  • Works with the onsite medical provider and helps coordinate appointments.
  • Assists residents with any transportation needs.
  • Routinely performs a med cart audit and checks in meds as needed.
  • Provides regular support to resident care needs and care-related follow-up as appropriate.
  • Ensures incident/fall reports are completed.
  • Continually seeks out and directs quality improvement activities that focus on wellness, clinical management, medication management, risk management, staff training, safety and survey preparedness.
  • Supports the hiring, training, and oversight of clinical care staff.
  • Ensures skin sheets, shower schedules, and charting are completed.
  • Routinely reviews call light times to ensure prompt responses.
  • Ensures compliance with all DHS 83 and 89 policies, procedures, and reporting requirements well as adherence to internal policies and procedures.
  • Maintains on-going communication with direct supervisor and other department heads on all notable resident service matters or changes.
  • When short staffed performs cares and passes medication.
  • Maintains knowledge of State Regulations and updates for DHS 83 and 89.
  • Assists with timecard corrections and payroll.
  • Available for on-call duties on an as-needed basis.
SUPERVISORY RESPONSIBILITIES

This position does have leadership responsibilities but does not have direct supervisory responsibilities except in the absence of the Executive Director. This position does participate in the performance management functions of the staff.
