Job summary

Qualifications

• Master's in social work or related field preferred with 1–2 years in aging services.
• Bachelor's degree required with 3–5 years of relevant experience.
• Compassion for and commitment to working with seniors.

Responsibilities

• Conduct wellness assessments and develop holistic wellness plans.
• Coordinate with primary care physicians, mental health providers and hospitals.
• Educate housing staff to identify and communicate changes in resident condition.
• Develop relationships with payers and coordinate with care teams.
• Lead care planning meetings and ensure documentation of interventions.
• Support residents transitioning across care settings and advocate for them.
