Job summary

Qualifications

• Masters in social work or related field with 1-2 years case management experience preferred.

• Comfort with documenting in electronic health/case management systems.

Responsibilities

• Conduct wellness assessments and develop comprehensive wellness plans for residents.
• Coordinate care with PCPs, mental health providers and hospitals.
• Educate housing staff and coordinate programming to meet resident needs.
• Document health information, end of life planning and emergency contacts.
• Support off-site services and ensure adherence to health activities.
• Lead care planning meetings and advocate for residents and families.
• Track metrics and generate reports on outcomes.
