Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks a Geriatric Wellness Coordinator to join the Supportive Housing Programs for adults and seniors. You will provide on-site wellness services to meet behavioral health and social needs of residents.

You will conduct interviews, crisis intervention, and ongoing therapy, develop service plans, coordinate care, and promote wellness activities to help residents age in place with dignity while maintaining permanent affordable housing.

Qualifications

• Bachelor’s degree in human services, Sociology, Psychology, Social Work or related field required.
• Master’s degree in psychology, Mental Health Counseling, Social Work preferred.

Responsibilities

• Provide supportive mental health counseling to tenants in need in the senior buildings.
• Meet with assigned residents on a regular basis and document progress with reference to service plan goals.
• Provide groups to seniors around benefits, entitlements, wellness and socialization.
• Facilitate psychoeducational workshops.
• Coordinate care and develop/support service plans for residents; crisis intervention and follow-up as needed.
