Job summary

Hebrew SeniorLife, Inc. in the Boston metro area is seeking a Wellness Coordinator to lead the R3 Initiative at Riverside Towers.

You will conduct wellness assessments, connect residents to services, and coordinate with nursing, housing staff and providers to support independent living. The ideal candidate is a social worker or care manager with aging or health care navigation experience; a MSW or related field is preferred, with strong communication and documentation skills and teamwork.

Qualifications

• Master's degree in social work or human services preferred; 1–2 years in case management for aging services beneficial.
• Bachelor's degree with 3–5 years experience required.
• Excellent organization and interpersonal skills; ability to manage multiple projects and work collaboratively.
• Experience documenting in health/social care software (e.g., AASC Online) is a plus.

Responsibilities

• Conduct wellness assessments of residents and develop wellness plans addressing physical, social, cognitive, spiritual and mental health domains.
• Coordinate with primary care physicians, mental health providers and hospitals.
• Partner with housing staff to tailor programming meeting resident needs.
• Educate housing staff to identify and communicate changes in residents' condition.
• Coordinate with residents to complete files including health information and care planning details.
• Support on-site services and connect residents to off-site services as needed.
• Develop relationships with payers serving seniors in the sites.
• Implement effective communication between housing and providers to relay important changes.
