Qualifications

Responsibilities

• Coordinate residents’ health-related services at site level.
• Build partnerships with health and wellness providers.
• Complete health screens and develop care plans.
• Facilitate resident navigation of health-related systems.
• Implement health-related group activities and events.

Skills

Familiarity with health issues in underserved communities

Case management experience

Strong communication skills

Bilingual Spanish preferred

Experience with chronic medical conditions

Education

Master’s degree in Human Services, Public Health, or related area
