Midland Care Connection is seeking a Wellness Assistant for the PACE program in Topeka, KS. The role focuses on promoting strength, mobility, and overall well‑being through tailored exercise plans and group activities. You will work with an interdisciplinary team to implement personalized care and maintain independence for participants.

Qualifications

• High school diploma or GED.
• Basic caregiving or healthcare experience.
• Ability to assist with activities of daily living (dressing, mobility, transfers).
• CPR and First Aid certification (or ability to obtain).
• Physical ability to lift and safely transfer participants.
• Ability to pass background checks and health screenings.

Responsibilities

• Promote strength, mobility and well‑being through individualized and group exercise programming.
• Lead and assist with therapeutic activities to improve balance, endurance and functional independence.
• Monitor participants' responses to exercise and report progress or concerns to the interdisciplinary team.
• Collaborate closely with therapy staff to support safe and effective wellness interventions.
