Job summary

Midland Care Connection is seeking a Certified Nursing Assistant to support older adults enrolled in the PACE program. You will assist with daily activities, monitor health, and work closely with a multidisciplinary team across center and home settings.

The role requires at least 1 year of direct care and a CNA or HHA credential, with CPR/First Aid and the ability to obtain certification if needed. The workweek is typically Monday through Friday, 8am–5pm, in the Topeka area.

Qualifications

• High school diploma or GED required.
• CNA or HHA certification, or ability to obtain certification within onboarding period per Midland policies.
• At least 1 year of direct care experience for older adults or chronic illness.
• Ability to assist with ADLs, bathing, grooming, dressing, toileting, transfers, and mobility.
• Follow safety practices, infection prevention and proper body mechanics in healthcare settings.
• Ability to follow individualized care plans and directions from IDT members (nursing, therapy).
• Strong communication and interpersonal skills with participants, families, and team members.
• Current CPR and First Aid certification, or ability to obtain on hire.
• Ability to pass background checks and health screenings.

Responsibilities

• Assist participants with daily activities such as bathing, dressing, grooming, and mobility.
• Monitor participants’ health, vital signs, and report changes to nursing staff.
• Assist with meals and accompany participants during activities.
• Provide support in both center and home settings to ensure comfort and independence.
• Follow care plans and collaborate with the interdisciplinary team.
