Job summary

Qualifications

  • Currently enrolled in an accredited Physical Therapy program (DPT).
  • Completed coursework in anatomy, physiology, and kinesiology.
  • Ability to follow clinical instructions and work under supervision of a licensed Physical Therapist.
  • Strong communication and interpersonal skills for working with older adults and interdisciplinary teams.
  • CPR/BLS certification (or ability to obtain before starting).
  • Compliance with health screenings and immunization requirements.

Responsibilities

Job Overview

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

PACE Program

Designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.

Responsibilities
  • Assist licensed physical therapists with patient evaluations.
  • Develop and implement individualized treatment plans.
  • Guide participants through therapeutic exercises aimed at improving mobility, strength, balance, and overall quality of life.
  • Collaborate with an interdisciplinary care team to address each participant’s comprehensive health needs.
  • Gain hands‑on experience in geriatric care, adaptive techniques, and patient‑centered service within a community‑based setting.
Hours

Flexible – to be determined with supervisor.

Pay Rate

$16.78 per hour.

Qualifications – Required
  • Currently enrolled in an accredited Physical Therapy (DPT) program.
  • Completion of foundational coursework in anatomy, physiology, and kinesiology.
  • Ability to follow clinical instructions and work under supervision of a licensed Physical Therapist.
  • Strong communication and interpersonal skills for working with older adults and interdisciplinary teams.
  • Basic knowledge of therapeutic exercise techniques and safety practices.
  • CPR/BLS certification (or ability to obtain before starting).
  • Compliance with health screenings, background checks, and immunization requirements.
Preferred
  • Prior clinical or volunteer experience in a rehabilitation, geriatric, or healthcare setting.
  • Interest in working with older adult populations, especially in community‑based care models like PACE.
  • Familiarity with electronic medical records (EMR) systems.
  • Strong organizational and time‑management skills.
  • Ability to adapt treatment approaches for individuals with complex medical conditions.
  • Demonstrated teamwork and problem‑solving abilities.
