Midland Care Connection in Topeka, KS, is looking for a PACE Nutrition Assistant to support the Registered Dietitian and help implement individualized nutrition plans for older adults in our PACE program.

You will monitor nutritional status, assist with screenings and documentation, help with menu planning and meal delivery, and work closely with the PACE team, Meals on Wheels, and other departments to ensure safety and dietary compliance.

Formación

• High school diploma or equivalent required.
• Strong organizational and communication skills.
• Ability to follow dietary instructions accurately.
• Ability to work with older adults and diverse diets.

Responsabilidades

• Monitor participants' nutritional status and report concerns.
• Assist with nutrition screenings and weight measurements.
• Maintain accurate documentation in the EMR.
• Support menu planning, food ordering, and special diets.
• Ensure safe meal preparation and delivery at the PACE Day Center.
• Collaborate with the Registered Dietitian, PACE team, and Meals on Wheels.
