Job summary

Step Up is seeking a Service Coordinator II to lead and coordinate clinical services for the VASH program in Los Angeles. You will supervise staff, ensure wrap-around services, and advocate for veterans with homelessness and mental health needs.

This exempt role requires independent judgment and collaboration with internal teams. You will participate in intake, coordinate housing/benefits and community resources, and manage client caseloads while upholding VA policies and trauma-informed care

Qualifications

• Master of Social Work, Marriage Family Therapist, or Psychologist licensed or eligible in CA.
• Two years of clinical and case management experience required.
• Own transportation and valid driver's license and insurance.
• Daily walking and occasional driving/transport of clients; ability to learn program EHR.

Responsibilities

• Establish collaborative relationships with service providers to ensure Veteran continuity of care.
• Advocate on behalf of the Veteran to assist in delivering needed services.
• Attend daily/weekly meetings, supervision and case conferences.
• Provide clinical and management support to staff and coordinate care of census.
• Assist with onboarding, training, and recruiting staff.
• Coordinate with team to ensure access to local resources including housing and benefits.
• Provide ongoing support and crisis intervention; manage a smaller client caseload as Service Coordinator II.
• Oversee documentation, maintain charts in CPRS, and ensure census accuracy.
