Job summary

Step Up seeks a licensed or credentialed case manager to support Veterans experiencing homelessness through housing assistance and clinical services. You will coordinate housing vouchers, provide case management, and link Veterans to VA facilities and community resources.

Successful applicants will have a bachelor’s degree in social sciences or related field, with master’s preferred, and at least five years of relevant experience. Occasional driving may be required.

Qualifications

• Bachelor's degree in social sciences or family therapy from an accredited institution.
• Five (5) years of demonstrated case management experience with high needs populations.
• Master's degree may reduce required case management experience to one year.
• Substance recovery certification may be required.

Responsibilities

• Lead and coordinate services to engage Veterans experiencing homelessness.
• Assist Veterans in navigating housing voucher processes and access permanent housing.
• Provide direct clinical case management and crisis support.
• Collaborate with VA providers and community agencies to ensure wrap-around care.
• Document care plans and maintain CPRS charts per VA requirements.
