Qualifications

• Education: High School Degree or equivalent.
• Experience: Must be 18+ and have a minimum of 1 year experience with frail elderly population.
• Licensure: Valid NC Driver's License with safe driving record and no major moving violations.

Responsibilities

• Coordinates and plans all transportation trips for Senior TLC daily attendance, specialty appointments, discharge from SNF and hospital, and dialysis trips.
• Determines safe and efficient pickup and delivery routes; communicates with drivers and care teams.
• Provides training and support to Drivers to ensure competence in operating the transportation fleet.
• Ensures drivers are trained on wheelchair lifts and tie-down equipment and operate them daily.
• Maintains timely delivery of participants to appointments and supports care planning per organizational protocols.
