Job summary

West PACE in California is seeking a Transportation Coordinator to manage comprehensive transportation services for PACE participants. You will liaise with participants, caregivers, and providers to ensure timely, safe access to medical appointments and the PACE center.

The role requires coordinating with vendors, maintaining schedules, and documenting activities in the EMR. A strong focus on compassionate care and collaboration with the IDT is essential.

Qualifications

• High School diploma or GED required.
• At least 1 year scheduling/transport coordination or related healthcare experience.
• Minimum 1 year experience with frail or elderly population.
• Experience using EMR and transportation or scheduling software preferred.
• Proficient in Microsoft Office applications (Outlook, Word, Excel).
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Coordinate and maintain transportation schedules to ensure timely access to PACE center and appointments.
• Serve as primary contact for transportation scheduling, changes, cancellations, and inquiries.
• Work with transportation vendors to resolve issues and improve service delivery.
• Document transportation activities and scheduling updates in EMR.
• Distribute daily schedules and maintain operational data for reporting.
