Child & Family Services Inc. is seeking a Senior Success Coach to provide intensive case management and family-centered, trauma-informed care in New Bedford or Fall River public middle schools.

The role includes intake interviews, service planning, advocacy, and psychoeducation, with travel to multiple settings as needed. The position requires a Master’s degree in Counseling, Social Work, or Psychology, experience with youth and families, and a commitment to culturally diverse populations.

Qualifikationen

• Master’s level degree in Counseling, Social Work, Psychology or related field.
• Experience working directly with youth, families, and substance use issues.
• Ability to work with culturally diverse populations.
• Must have a valid driver’s license and reliable transportation.
• Strong communication and documentation skills.

Aufgaben

• Provide a safe, welcoming environment for clients and families.
• Perform intake interviews, needs assessments, and individual service planning.
• Deliver psychoeducation and advocacy as appropriate.
• Conduct regular case management meetings with parents/caregivers and stakeholders.
• Maintain complete documentation of meetings, releases, and referrals.
• Develop youth development plans and coordinate services with community resources.
• Travel to various settings to deliver services and attend meetings.
