Job summary

Priority Management is seeking a motivated Social Worker to lead and coordinate social services in a Skilled Nursing Facility. In this in-person role, you will assess resident needs, plan and implement social care programs, and collaborate with medical staff to support residents and families.

You will interview residents and families, participate in discharge planning, arrange transportation, refer to community resources, and help develop policies.

Qualifications

• Skilled Nursing Facility experience preferred
• Licensed Social Worker preferred
• In-person work required

Responsibilities

• Plan, develop, organize, implement, evaluate and direct social service programs
• Develop and implement policies for identifying medical and emotional needs
• Participate in discharge planning and social care assessments
• Interview residents/families to obtain family history
• Complete required forms and reports to the Director
• Involve residents and families in planning social service programs
• Arrange transportation to other facilities as needed
• Refer residents to appropriate social service agencies
• Coordinate social service activities with other departments
• Interpret needs to medical staff and care team
• Review complaints and report actions to the Administrator
