AuthentiCare Health Centers is seeking a Social Services Director to lead resident-centered social work in our skilled nursing setting. You will guide discharge planning, family support, and care planning with a compassionate, collaborative team.

The role requires a MA-licensed Social Worker with geriatric care experience; strong communication, confidentiality, and organizational skills are essential for day-to-day leadership and coordinating with the healthcare team.

Qualifications

• Licensed Social Worker in MA is required.
• Experience with geriatric residents is preferred.
• Two years in long-term care or healthcare settings preferred.
• Experience with Alzheimer’s/Dementia is a plus.
• Strong communication and confidentiality.

Responsibilities

• Conduct discharge planning, behavior, and care plan meetings.
• Assist residents and families with coping with the new environment and changes to physical or mental capacity, end-of-life concerns, and the grieving process.
• Work directly with the patient, family, and other team members to outline the goals at admission and create a plan to meet those goals.
• Responsible for assessing our resident's psychological status during admissions and other intervals during residence.
• Assessment of our residents' psychological status during Admission.
• Knowledgeable in PASSRS, MDS, Annuals, and Quarterlies during their residency.
• Collaborate with healthcare team members to deliver comprehensive resident care in our facilities.
• All other duties as assigned.
