Job summary

M2 Healthcare is seeking a compassionate Social Worker to join our interdisciplinary team in a skilled nursing facility. You will assess residents, develop care plans, and provide counseling to residents and families to support well-being and transitions of care.

The role emphasizes advocacy, regulatory compliance, and collaborative care within a dynamic team, with opportunities for professional growth and involvement in QAPI initiatives.

Qualifications

• Bachelor's degree in a related field is required.
• Current state licensure or eligibility (if applicable).
• Experience in long-term care, skilled nursing, or healthcare setting preferred.
• Strong communication, organizational, and interpersonal skills.
• Knowledge of federal and state long-term care regulations is preferred.
• Ability to collaborate within an interdisciplinary team.

Responsibilities

• Complete psychosocial assessments for residents upon admission and as needed.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning, referrals, and coordination of community resources.
• Advocate for resident rights and ensure compliance with regulatory standards.
• Facilitate family meetings and care conferences.
• Support residents with adjustment to illness, disability, and long-term care placement.
• Maintain accurate and timely documentation in accordance with facility policy and regulations.
• Assist with grievance resolution and conflict mediation.
• Participate in QAPI and facility improvement initiatives.
