Job summary

The Social Services Director leads our social services team within a skilled nursing facility, coordinating with healthcare professionals to provide comprehensive care. This role emphasizes program development, counseling, case management, and discharge planning to ensure high-quality resident outcomes.

You will work to support residents’ social and emotional well-being, supervise staff, and build partnerships with community resources to enhance services.

Qualifications

• Master's Degree in Social Work
• Bilingual Spanish preferred
• Experience managing Social Service Aides
• Experience with residents in a skilled nursing facility with behavioral/psychiatric needs

Responsibilities

• Develop and implement social services programs that cater to the needs of our residents, including counseling, case management, and discharge planning
• Manage a caseload of residents, providing individualized support and guidance to ensure their social and emotional well-being
• Collaborate with healthcare professionals to develop and implement comprehensive care plans that address the physical, emotional, and social needs of our residents
• Coordinate with community resources and outside agencies to provide additional support services to residents
• Participate in multi-disciplinary care conferences to ensure seamless care and communication among team members
• Recruit, train, and supervise social services staff to ensure excellent service delivery
• Monitor and evaluate program effectiveness, making recommendations for improvements
• Develop and maintain relationships with community partners and stakeholders
