OPCO Skilled Management seeks a licensed Social Worker to identify psychosocial, mental, and emotional needs of residents in a skilled nursing setting. You will assess residents, develop care plans, and ensure services comply with NASW ethics and regulatory requirements.

You will collaborate with an interdisciplinary team to support resident well-being, coordinate discharge planning, and educate families about care options to improve overall quality of life.

Qualifications

• Bachelor’s Degree in Social Work or Human Services field from an accredited school.
• One year of supervised social work experience in a healthcare setting with geriatric residents.

Responsabilités

• Complete social history and psychosocial assessment for each resident and guide the care plan using the MDS.
• Maintain accurate documentation per federal/state regulations and company policy, including care plans and psychosocial notes.
• Support residents' rights and inform about therapeutic alternatives; involve others to educate about options.
• Develop interdisciplinary care plans that address psychosocial needs and goals for residents.
• Coordinate discharge planning and referrals for home care services when returning to the community.
• Collaborate with the nursing and administration team to promote resident well-being and comply with inspection standards.
• Identify and address resident abuse concerns as mandated by law and licensure.
• Assist families and residents in self-advocacy to improve quality of life and care.
