Job summary

Opco-Skilled-Management- seeks a dedicated Social Worker to identify psychosocial, mental, and emotional needs of residents in a Skilled Nursing Facility, delivering access to needed services in line with the NASW Code of Ethics and applicable regulations.

The role involves conducting social histories, coordinating interdisciplinary care plans, supporting resident rights, and guiding families through placement and service options with meticulous documentation and compliance.

Qualifications

• Certified, licensed or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work, required.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs; uses MDS to guide the care plan.
• Maintains accurate documentation compliant with federal/state regulations and center practices, including directives, care plans, progress notes and discharge planning.
• Supports residents’ right to self-determination and informs about therapeutic alternatives when needed.
• Develops an interdisciplinary plan of care addressing psychosocial needs and appropriate interventions.
• Provides therapeutic support to help residents cope with transition to long-term care and adjusts they needs.
• Educates residents and families about placement issues and refers to social service agencies when needed.
• Coordinates discharge planning and refers to home care services prior to returning to the community.
• Works with the team to protect resident rights and well-being; addresses abuse per law and licensure.
• Assists in advocating for residents to enhance quality of life within facility and community.
