Blue Shield of California Promise Health Plan seeks a Senior Social Worker to conduct assessments of social drivers of health and provide intensive interventions. You will link members to community resources to remove barriers and help them access and manage medical care, supporting stable living in the community.

The role emphasizes experience with Medi-Cal populations and knowledge of state and federal programs; hybrid virtual work is available with in-office requirements as needed.

Qualifications

• Master's in Social Work from an accredited university required.
• LCSW or equivalent licensure preferred.
• CCM certification preferred.
• 3+ years of Medi-Cal or medical social work experience preferred.

Your Role

The Social Worker, Senior reports to the Manager, Social Services Management. In this role you will outreach to Blue Shield of California Promise Health Plan members and conduct assessments for social drivers of health needs and provide intensive interventions. The Social Worker, Senior provides linkages to community resources to reduce social barriers, which enable members to receive and manage their medical care as well as remain living stable in the community. This position requires experience working with the Medi-Cal population and knowledge of community resources and related interventions.

Your Knowledge and Experience

• Master's Degree in Social Work (MSW) or related field from an accredited university required
• Licensed Clinical Social Worker (LCSW) or equivalent licensure (e.g. LPCC, LMFT, etc.) preferred
• Case Manager Certification (CCM) preferred
• 3 or more years of experience working with the Medi-Cal population, medical social work, community clinics, or other home and community-based organizations
• Strong analytical and problem-solving skills
• Experience conducting thorough psychosocial assessments, developing care plans and providing necessary interventions identified during assessment with complex client populations
• Thorough knowledge of services provided by, and eligibility for, applicable state and federal programs, including Long Term Services and Supports (LTSS), in addition to local, community-based organizations
• Proficient skills with computer programs such as Microsoft Excel, Outlook, Word, and PowerPoint
• Excellent communication skills both orally and in writing
• Knowledge of health disparities in diverse communities
• Effective time management skills and ability to define and act on priorities amid multiple responsibilities
• Bilingual Spanish speaking preferred

This role allows employees to work virtually full-time, however employees will be expected to come to the office based on business need.
