Qualifikationen

• Bachelor's degree in social work from an accredited institution is required.
• A human services degree with related experience may substitute in some cases.
• Equivalent combination of education and experience will be considered.

• Interviews clients and family members to determine risk factors.
• Observes behavior to determine family dynamics.
• Identifies strengths and weaknesses in clients’ support systems.
• Communicates and elicits expression to understand client’s perception of problem.
• Analyzes socio-economic and environmental factors affecting client’s situation.
• Develops service plans with clients and reviews progress.
• Monitors progress and adjusts plans as needed.
• Coordinates with medical and other service providers.
• Provides counseling and advocacy to support client self-sufficiency.
• Intake, eligibility determination, and coordination with SA/IH referrals.
