Job summary

Qualifications

• Must possess, at minimum, an LBSW from an approved school of Social Work.
• Must have one year supervised social work experience in a health care setting.
• Ability to make independent decisions when circumstances warrant.
• Ability to interact tactfully with residents, families, staff and the public.

Responsibilities

• Plan, develop, organize, implement, and evaluate social services programs.
• Develop and coordinate department policies and procedures.
• Identify and address medically related social and emotional needs.
• Participate in discharge planning and care plan development.
• Interview residents or families to obtain social history.
• Refer residents to appropriate social service agencies when needed.
• Coordinate social services with other facility departments.
