Job summary

The State of Nevada Aging and Disability Services Division (ADSD) is recruiting a Social Worker 2 in Reno, NV. The role supports adults and vulnerable individuals in APS and HBCS, emphasizing coordinated case management, home visits, and individualized care planning.

Under supervision, you may underfill a Social Worker 3 with APS. The position requires licensure as a Social Worker and at least one year of relevant professional experience in case management, with duties spanning investigations,

Qualifications

• Licensure or provisional licensure as a Social Worker and one year of professional experience providing case management in a social work setting equivalent to Social Worker I in Nevada State service.

Responsibilities

• Assess client levels of functioning through interviews and assessments.
• Develop and implement individualized case plans with services.
• Coordinate services with state agencies and community partners.
• Provide ongoing case management including home visits and progress tracking.
• Organize MDT meetings to plan protective services as needed.
• Document activities and prepare case reports.
