Job summary

Aging and Disability Services Division in Reno, Nevada, is recruiting for a Social Worker 2 to support Adult Protective Services and House & Community Base Services. The role involves eligibility determinations, care planning, home visits, and documenting in web-based case management systems.

Qualifications

• Licensure or provisional licensure as a Social Worker or related field with at least 1 year of professional case management experience.
• Experience conducting interviews, assessments, and developing care plans.
• Ability to document accurately and maintain records.

Responsibilities

• Determine eligibility, develop and implement individualized care plans.
• Conduct home visits and assessments to coordinate supportive services.
• Coordinate with state agencies, community organizations, and private providers.
• Maintain case documentation and ensure compliance with policies and regulations.
• Organize MDT meetings to support protective services and planning.
• Provide crisis intervention and act as liaison with authorities when needed.
