Job summary

The Aging and Disability Services Division (ADSD) in Elko, Nevada, is recruiting for a Social Worker 2. This journey‑level position delivers case management, eligibility determinations, and care‑planning for at‑risk elderly and people with a disability within the Office of Community Living (OCL).

You will perform home visits, coordinate services with state agencies and community partners, document progress, and guide clients toward community integration while helping to prevent

Qualifications

• Licensure or provisional licensure as a Social Worker, Clinical Social Worker, Independent Social Worker or Associate in Social Work and one year of professional experience providing case management in a social work setting equivalent to a Social Worker I in Nevada State service.
• Strong knowledge of social work principles, practices, and case management.
• Ability to conduct home visits and coordinate services with multiple agencies.

Responsibilities

• Assess and reassess client functioning through interviews, records, and tests to determine needed services.
• Develop and implement a case plan and coordinate resources across agencies.
• Coordinate a network of services from state agencies, community organizations, and private providers.
• Provide ongoing case management including home visits, monitoring services, and documentation.
• Lead or participate in MDT meetings for complex cases and protective services planning.
• Document case activities and prepare reports for law enforcement and courts.
