Aging and Disability Services Division (ADSD) in Las Vegas is seeking Social Worker staff at various levels to support APS and HCBS programs. The position involves evaluating needs, planning interventions, and ensuring client safety through coordinated services.

Social Workers conduct home visits, document case progress, and may supervise other staff while advancing through the trainee levels based on performance and qualifications.

Qualificações

• Licensure or provisional licensure as a Social Worker, Clinical Social Worker, Independent Social Worker, or Associate in Social Work.
• Understand and apply case management principles and documentation
• Ability to assess client needs and plan services with relevant stakeholders.

Responsabilidades

• Perform eligibility determinations, care planning, home visits, and development of individualized care plans.
• Maintain documentation using web-based case management software.
• Investigate reports of abuse, neglect, exploitation, isolation and abandonment involving vulnerable adults.
• Coordinate services with independent providers and other state agencies.
• Provide crisis intervention and assist with follow-up case management as required.
