Job summary

The Agency on Aging of South Central Connecticut (AOASCC) seeks a Senior Services Specialist-Service Navigator to help older adults, individuals with disabilities, and caregivers access health and social services.

This community-based role includes a four-day-per-week presence in a local hospital Emergency Department, with one day in the office or remote to support follow-up, care coordination, and documentation.

Qualifications

  • A Bachelor of Social Work is required; a MSW is strongly preferred.
  • Three (3) years of Case Management Experience.
  • Strong communication and interpersonal skills.
  • Empathy and compassion for older adults and individuals with disabilities.
  • Ability to work independently and as part of a team.
  • Familiarity with community resources and benefit programs.
  • AIRS certified or eligible to sit for certification AIRS Certification within 120 days of hire
  • SHIP/CHOICES certified.

Responsibilities

  • Provide personalized assistance to older adults, individuals with disabilities, and caregivers as they navigate health and social service systems.
  • Identify patients in the Emergency Department and connect with community-based services as needed.
  • Develop plans for safe transitions back to the community and to prevent readmissions.
  • Assist clients with accessing health insurance, Medicare, Medicaid, Long-Term Care, and other resources.
  • Support care coordination and collaboration with community partners.

GENERAL JOB DESCRIPTION:

As a Senior Services Specialist-Service Navigator, you will be a vital part of the Aging Answers team. Your role involves providing personalized assistance to older adults, individuals with disabilities, and caregivers as they navigate complex health and social service systems. While the position includes traditional telephone-based and office support, this is a community-based position with a primary presence in a local hospital Emergency Department four days per week. The fifth day will be spent in the office or remotely to support follow-up, care coordination, documentation, and collaboration with community partners.

Working directly in the Emergency Department, the Service Navigator will identify older adults and caregivers who may benefit from community-based services and provide timely, person-centered support during and following an emergency department visit. You will help individuals understand their options, connect with appropriate resources, and develop plans that support safe transitions back to the community and help prevent avoidable hospital readmissions or unnecessary institutional placement.

Through both Emergency Department and Aging Answers activities, you will guide clients and caregivers in accessing essential services related to health insurance, Medicare, Medicaid, Long-Term Care, and other health, social, and community resources. The goal is to alleviate pressure on existing staff, improve care coordination and workflow efficiency, address unmet needs, and empower older adults and caregivers to remain safe, independent, and engaged in their communities.
