VOA Colorado is seeking a Meals on Wheels Program Coordinator I in Commerce City, CO to coordinate meal service for at-risk seniors and connect them with needed services. You will maintain intake, support eligibility and enrollment, and link clients with housing, SNAP and healthcare while handling a 300-500 client caseload.

This full-time, in-person role requires strong communication and integrity in handling confidential records.

Qualifications

• Ability to work independently and as part of a multidisciplinary team.
• Strong organizational and time management skills.
• Professional communication with clients, volunteers, staff, and partners.
• Ability to handle sensitive and confidential information.
• Proficiency in Microsoft Office and ability to learn program databases.

Responsibilities

• Establishes trusting relationships with coworkers and participants.
• Collaborates with the team to serve program participants effectively.
• Maintains intake, reassessments, and ongoing referrals.
• Answers calls and assists with eligibility and enrollment processes.
• Connects clients to housing, SNAP, and healthcare resources.
• Maintains a caseload of 300-500 clients with confidential records.
• Coordinates Medicaid services and acts as primary client contact.
• Handles crises professionally and reports incidents as required.
• Maintains confidentiality of participant information.
• Assists with daily duties and supports client routes when needed.
• Communicates with team to ensure accuracy and efficiency.
• Researches resources and provides meaningful referrals.
