Job summary

The Unity Council is seeking a Senior Services Coordinator to help older adults stay healthy, connected, and independent. You will assess needs, coordinate resources, and link clients with benefits and community services to promote safety, wellness, and housing stability.

Collaborative work with partners supports aging in place with care and dignity. Ideal candidates have 1–2 years in resident or social services, strong communication, and comfort navigating Medicare, Medi-Cal, Social Security,

Qualifications

• High School Diploma or GED required; advanced degree preferred.
• 1–2 years of resident services or social services experience.
• Familiarity with Medicare, Medi-Cal, Social Security, IHSS.
• Strong communication, problem-solving, and organizational abilities.

Responsibilities

• Provide general assistance, advocacy, and referrals for older adults.
• Conduct basic needs assessments for health, safety, and housing stability.
• Assist with benefits navigation (Medicare, Medi-Cal, Social Security, IHSS).
• Coordinate referrals for in-home services, transportation, and meals.
• Document interactions, referrals, and outcomes accurately.
• Support planning and delivery of on-site wellness and education activities.
• Engage with community partners to promote aging in place with dignity.
