Job summary

Selfhelp Realty Group is seeking a dedicated case manager to provide SHASAM services to residents, supporting housing stability and reducing social isolation. This role requires compassionate counseling, strong communication, and meticulous record-keeping.

Responsibilities include conducting intakes, guiding clients to benefits, developing care plans, maintaining case notes, and coordinating services with community partners. On-site work in New York region with a focus on dignified service.

Qualifications

• Bachelor’s Degree required.
• One year related experience and/or training.
• Working knowledge of Microsoft Office Suite and other technology.
• Excellent communication and listening skills.
• Excellent customer service skills with a focus on treating clients with respect and dignity.
• Able to multi-task and work independently with great attention to detail.

Responsibilities

• Conduct intakes and screen clients with presenting problems, if required.
• Provide information on benefits, entitlements and assist in accessing services.
• Apply and recertify clients for entitlements and benefits.
• Arrange for the provision of services.
• Maintain complete and accurate case notes in the program’s data system.
• Develop, implement and follow up on a care plan for clients.
• Provide supportive counseling and ongoing contact with clients.
• Supervise social work students, as needed.
• Exchange information in a timely manner and participate in supervision.
