As a Part-Time Case Manager, you will provide essential social services to older adults and their families in accordance with professional social work standards. You will be responsible for conducting thorough assessments, developing personalized care plans, and serving as a dedicated advocate to ensure our clients maintain their autonomy and receive the full range of benefits and services they deserve.

What You’ll Be Doing

You will manage all aspects of social casework, from initial assessment and counseling to long-term monitoring and crisis intervention.

Key Responsibilities:

  • Clinical Assessment: Utilize standardized instruments (e.g., PSA, EISEP-PATH) to assess the social and emotional needs of older adult clients and their families.
  • Case Planning & Counseling: Develop and implement written case plans; provide individual and family counseling to help resolve social, physical, or psychological challenges.
  • Benefit Advocacy: Screen clients for eligibility and assist with applications for entitlements (Medicare, Medicaid, SNAP, etc.) while providing necessary advocacy.
  • Service Coordination: Arrange for direct services including homecare, transportation, meals on wheels, legal aid, and psychiatric examinations.
  • Protection & Safety: Evaluate clients' capacity to manage their affairs; identify signs of abuse or exploitation and make referrals to Adult Protective Services as needed.
  • Community Education: Conduct group counseling and community education groups; represent and interpret JASA’s mission at community conferences.
  • Monitoring & Reassessment: Conduct ongoing visits to evaluate service efficacy, monitor risk, and revise case plans as client needs evolve.
  • Administration: Maintain accurate case records, reports, and statistics; participate in unit meetings and required trainings.
What You’ll Need
  • Education: Graduate of an accredited college or university with a BSW, BA, or BS degree (or equivalent).
  • Technical Skills: Computer literacy with the ability to learn and use program-specific software; strong record-keeping skills.
  • Communication: Ability to read, write, and speak English; knowledge of Spanish is helpful.
  • Travel: Ability to travel to clients' homes using mass transit and navigate physical barriers (stairs, etc.) in residences.
