Job summary

Qualifications

• Bachelor's degree in Social Work or a related field.
• Minimum of five years of experience in social services.
• Experience with geriatric populations preferred.

Responsibilities

• Conduct assessments to identify residents' social, health, and support needs.
• Provide case management, resource referrals, and service coordination.
• Coordinate community services such as transportation, meal programs, counseling, wellness programs, and healthcare services.
• Build and maintain relationships with local agencies and community partners.
• Advocate for residents and help them navigate applications, services, and benefits.
• Develop individualized service plans and monitor ongoing support services.
• Promote resident-centered engagement and coordinate educational programs.
