Job summary

Rise Services Inc. in Missouri is seeking a Community Support Coordinator to oversee the delivery of community support services.

You will supervise direct care staff, help hire and train team members, and ensure services are provided with accuracy and empathy to consumers. The role requires a Bachelor’s degree in a human services field, plus at least four years of direct care experience with individuals who have developmental disabilities.

Qualifications

• Bachelor's degree with 24 earned credit hours in Special Education, Early Childhood Education, Psychology, Social Work, Sociology, Counseling, Recreation (including specialties such as Art, Dance, Music, or Physical Education), Speech-Language Pathology or Audiology
• 4 years of full time paid experience providing direct care to persons with developmental disabilities as defined by DMH, or a combination of the two.
• QDDP relevant experience preferred.

Responsibilities

• Supervise and train direct care staff and participate in interviewing and hiring.
• Monitor program implementation with daily documentation review and data collection.
• Manage staff performance, ensure productivity and engage participants with community opportunities.
• Implement Individual Support Plans and report progress to the interdisciplinary team.
• Maintain high professional standards and ongoing professional development.
• Maintain client and agency files per funder and accrediting requirements.
• Familiarize self with Agency Policies and Procedures.
