Qualifications

• Candidates must meet NYS Service Coordinator qualifications.
• Minimum one year of case management/service coordination experience.
• Experience as an NHTD/TBI Service Coordinator preferred.

Responsibilities

• Supervise Service Coordinators according to state regulations.
• Review and approve Individual Service Plans for compliance.
• Conduct periodic file audits and case reviews for quality assurance.
• Provide training and mentoring to Service Coordinators.
