Priority Cares Home Services (PCHS) is a New York State Licensed Home Care Services Agency (LHCSA) dedicated to helping individuals live independently and with dignity in their own homes and communities. We serve clients across all five NYC boroughs as well as Rochester and Monroe County, providing compassionate, person-centered care through the NHTD Waiver, TBI Waiver, Managed Long Term Care (MLTC), and Personal Care Aide (PCA) programs.

The Service Coordinator for the NHTD/TBI Waiver Program is a critical member of the Priority Cares Home Services team, responsible for managing the care coordination needs of individuals enrolled in the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid Waiver programs. The Service Coordinator serves as the primary point of contact for participants, their families, and all service providers, ensuring that each individual receives the supports they need to live safely and successfully in the community.
