Job summary

A non-profit organization in Albany is seeking a Service Coordination Supervisor to lead and support Service Coordinators in providing person-centered services for individuals with disabilities and seniors. The ideal candidate will have a LMSW or LCSW and at least a year of experience in case management. Qualifications

• Minimum one year of case management/service coordination experience, including referrals and linkage to community services.
• Experience as an NHTD/TBI Service Coordinator is strongly preferred.
• Valid NYS driver’s license and reliable transportation required.

Responsibilities

• Supervise Service Coordinators in accordance with state regulations.
• Conduct periodic file audits and case reviews to ensure compliance.
• Provide training, mentoring, coaching, and performance feedback.

Skills

Strong understanding of community-based supports and service systems

Leadership skills in supervision, mentoring, and performance management

Excellent interpersonal, communication, and organizational abilities

Proficiency in Microsoft Office, data entry, and EHR systems

Education

LMSW or LCSW

Master’s/Doctorate in Social Work, Psychology, or Gerontology
