Job summary

Driscoll Children’s Hospital is seeking a Service Coordinator III in Corpus Christi, TX, to support care coordination for pediatric LTSS and STAR Kids members. You will work with physicians, nurses, social workers, and care teams to ensure timely access to community-based services and to promote independence and wellbeing.

Qualifications

• Experience in case management support or a medical office/health plan setting.
• Knowledge of STAR Kids and LTSS programs preferred.
• Confidentiality and regulatory compliance are essential.
• Knowledge of Texas Medicaid criteria is helpful.
• Proficiency with Microsoft Office and data management.

Responsibilities

• Provide administrative support for care coordination to the assigned care team.
• Coordinate Level 3 STAR Kids outreach and member services as warranted.
• Communicate with physicians, social workers, nurses and other team members to optimize outcomes.
• Produce regular status reports and monitor workload and turnaround times.
• Ensure compliance with STAR Kids contract requirements and health plan policies.
