Driscoll Health System seeks a Service Coordinator III to support care coordination and LTSS for STAR Kids members. You will collaborate with RN/SW teams, manage referrals, and assist in planning services to maintain independence and community-based care for medically complex children.

The role emphasizes administrative support, policy development, and cross-disciplinary communication to achieve timely, cost-effective outcomes for pediatric patients and their families.

Qualifikationen

• Experience in case management support or LTSS coordination preferred.
• Experience with STAR Kids or similar programs helpful.
• Experience with managed care environments preferred.

Aufgaben

• Provide administrative support for care coordination.
• Coordinate care and utilization of health care resources for STAR Kids members.
• Assist in the development of patient care policies.
• Operate Service Coordination Pod communications and track workload.
• Support outreach to Level 3 STAR Kids members as assigned.
